Trojan Band Student Information Form

    Please complete thus form and submit with your payments by July 18th. Thank you.
    This form must be completed for the Band Directors' use. Further, the information on this form will also be used to create a band directory. This directory includes members from the band and dance guard. This directory is intended for the sole use of Trojan Band business and not for use for solicitations, charitable causes, etc.
  • Please enter a value between 9 and 12.
  • We will find one for your if you leave this blank.
  • A Medical Release and Private Transportation form is REQUIRED in order for AHS Band Students to participate in band activities outside of regular school hours, or at off-site locations. The FORM may be downloaded from the above link, signed, scanned, and re-attached here if convenient. This form can also be turned in to any AHS Band Director.

  • Student Uniform Orders

  • New Students will receive a pair free of charge. Returning band members who need new shoes for $25.
    All band members will receive a "Show Shirt" (black tee shirt) for Tango. Please indicate your tee shirt size.
    All incoming band members will receive a polo shirt free of charge. Returning band members who new shirts will be charged $20.
